Important Information about Procedures for Opening a New Account
To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.
What this means for you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
To open a new account, please use the link ‘Donor-Advised Fund Application’ below. If you are having difficulty viewing or completing the application form online, or, if the process is not working to your satisfaction, please download the form using the ‘Download’ link below, print and complete the form, and mail it to us, along with your initial gift to: Deseret Trust Company, 60 East South Temple, Suite 800, Salt Lake City, Utah 84111-1036 Please note the ‘Download’ version of the application requires the use of Adobe Reader.
